Charlin Health Services is seeking a detail-oriented Home Health Intake Coordinator to support patient admissions and referral processing. This role serves as a key point of contact for patients, referral sources, and healthcare providers while ensuring accurate documentation and timely coordination of services.
Key Responsibilities
- Receive and process patient referrals
- Verify demographic, insurance, and referral information
- Coordinate patient admissions and scheduling
- Communicate with physicians, hospitals, and referral partners
- Maintain accurate patient records and documentation
- Provide professional customer service to patients and families
- Ensure compliance with HIPAA and company policies
Qualifications
- High School Diploma or equivalent
- Previous healthcare, medical office, or administrative experience preferred
- Strong communication and organizational skills
- Proficiency with Microsoft Office and data entry systems
- Ability to manage multiple tasks in a fast-paced environment
